Sildenafil not working? What to check

The cited patient leaflet identifies food and sexual stimulation as factors that can affect sildenafil response, but it does not establish how often they explain an unsatisfactory response. These factors do not explain every case, so repeated failure needs a prescriber review. The PIL states a high-fat meal delays absorption, you still need sexual stimulation, and the dose must not be increased or combined with other ED medicines without supervision. If it fails after several attempts, speak to your prescriber.

Sildenafil is a sensitive health topic, so we explain it directly and without shaming. Prepared by Testovic, editorial author, without a documented health qualification and without an independent professional reviewer.

Warning: this article is for information only and does not replace a doctor or pharmacist. It does not recommend a dose, a product or a purchase. If you take nitrates or have a heart condition, do not experiment — speak to your prescriber first.

Timing, food and the PIL all influence whether sildenafil works as expected.

Sildenafil not working or only delayed? Tell the difference

A delayed onset does not by itself identify the cause. The March 2026 UK PIL notes that taking it with food may delay the effect. That is different from a genuine lack of response, where the medicine has had adequate time and stimulation but produces no meaningful erection. The first is solved by timing; the second needs a clinical review.

Sildenafil not working on a single occasion, especially after a heavy meal or a rushed attempt, is therefore not yet a treatment failure. However, if the pattern repeats on separate days with correct timing, treat it as a question for your prescriber.

Food, timing and arousal — the three prerequisites

The PIL is explicit on all three:

  • Food: A heavy or high-fat meal can slow absorption. Taking sildenafil on an empty stomach or after a light meal gives the most predictable timing.
  • Timing: You typically take the tablet 30–60 minutes before anticipated sexual activity. It does not work instantly.
  • Arousal: Sildenafil only works with sexual stimulation. For example, without arousal there will be no erection, because the medicine does not create one on its own.

These points are covered in our sildenafil complete guide alongside dosing ranges and interaction summaries.

When sildenafil is not working, do not raise the dose yourself

Sildenafil not working is not a reason to double the dose. The PIL warns: do not take more than one dose in 24 hours and do not use with other ED treatments. Increasing the dose or adding another PDE5 inhibitor raises the risk of hypotension, prolonged erection, vision changes and cardiovascular events without improving efficacy. The EMA’s Viagra EPAR confirms the maximum authorised dose is 100 mg once daily. Any dose change must be directed by your prescriber.

What an observational table shows — and what it does not

Below is a compact comparison of the evidence tiers we consulted for this article. It is an editorial methodology table, not a clinical decision tool.

Editorial note: This comparison was performed by Testovic (author) on 17 September 2026. No independent doctor or pharmacist reviewer has been documented. The table distinguishes a registered medicine’s approved leaflet from catalogue claims — a catalogue entry does not inherit the PIL’s authority.

Sildenafil not working after several tries: what to record for your prescriber

Bringing a short, factual record makes the consultation faster and safer, because your prescriber can rule out the simple causes first. Note:

  1. Product name and strength exactly as on the pack (for example “Viagra 50 mg”, not “the blue pill”).
  2. Dose taken and time before activity.
  3. Meals in the 3 hours before dosing (light / heavy / fatty).
  4. Stimulation — was there adequate sexual arousal?
  5. Number of attempts on separate days (the PIL suggests trying on several occasions).
  6. Other medicines — especially nitrates, nicorandil, riociguat, alpha-blockers, antihypertensives, antifungals (ketoconazole, itraconazole), HIV protease inhibitors, macrolide antibiotics.
  7. Health conditions — cardiovascular disease, recent stroke or heart attack, severe hepatic or renal impairment, retinitis pigmentosa, anatomical penile deformation, priapism history.
  8. Effect observed — none, partial, delayed, lost during activity.
  9. Side effects — headache, flushing, dyspepsia, nasal congestion, visual disturbance, dizziness.

A concise record helps your prescriber decide the next step.

Urgent warnings — seek medical help immediately

Stop taking sildenafil and get emergency care if you experience:

  • Chest pain during or after sexual activity — do not use nitrates to treat it.
  • Sudden vision loss in one or both eyes (possible non-arteritic anterior ischaemic optic neuropathy).
  • Erection lasting more than 4 hours (priapism) — this is a medical emergency to prevent permanent damage.

These warnings come directly from the approved PIL and the EMA assessment. They are not negotiable and no brand variation changes them.

Frequently asked questions

What stops sildenafil from working?

A heavy meal can delay sildenafil’s onset, and sexual stimulation is needed for it to work — the approved leaflet states both. The effect also needs time, normally 30 to 60 minutes. These points do not explain every lack of response: if the medicine repeatedly does not help, follow your leaflet and ask your prescriber to review it. Nitrates and riociguat are dangerous combinations for blood pressure, not remedies for poor response.

What do I do if sildenafil does not work?

Try it on several separate occasions with a light meal and adequate stimulation. If it still does not help, contact your prescriber. Do not increase the dose or add another ED medicine. Your doctor can review the diagnosis, check interactions, and consider an alternative PDE5 inhibitor or a different treatment pathway.

How can I make sildenafil more effective?

Take it on an empty stomach or after a light meal, allow 30 to 60 minutes before activity, ensure sexual stimulation is present, avoid alcohol excess, and do not combine with other ED treatments. These steps follow the authorised PIL. Any further adjustment requires medical supervision.

Can you become resistant to sildenafil?

The authorised literature does not establish clinical tolerance to the pharmacological action of sildenafil. This general guide cannot identify the cause of a change in an individual’s response. Discuss changes with your prescriber.

Is there anything stronger than sildenafil?

Stronger is not a precise clinical concept. Tadalafil and vardenafil are different PDE5 inhibitors with distinct onset, duration and food-effect profiles. Our sildenafil vs tadalafil comparison sets out the sourced differences without recommending one over another. The choice depends on your medical history, dosing preference and prescriber judgment.
